Digital Access to Health Information and Application Programming Interfaces (APIs)
Per federal and state requirements, a set of Application Programming Interfaces (APIs) have been made available for you to access your mental health and/or substance use disorder service health records maintained by Tuolumne County Behavioral Health Department. As a recipient of services, you and/or your legal representative have a right to view your records and/or request a copy be sent to you or a third party. You can view your records electronically or request printed versions of the information.
Patient Access API is the system for you or your authorized representative to access specific patient records managed by Tuolumne County Behavioral Health. It’s important to note, while these systems are freely available, they do require a level of knowledge on Qualifacts system and proficiency to efficiently utilize them.
Currently, Tuolumne County Behavioral Health is not partnered with a third-party application to use these APIs. However, when we do, you and your authorized representative can feel confident in the safety and privacy standards of the application that accesses your Tuolumne County Behavioral Health Patient Access API because our department will have reviewed and approved the application.
Patient Digital Access to Medical Record
You or your authorized representative can view your records electronically at Qualifacts Credible Systems.
If you know of an app to use to connect to other medical records and you would like it to interface with our system, please notify behavioralhealth@tuolumnecounty.ca.gov.
Additionally, if you are an app developer and wish to interface with our patient access API, please contact behavioralhealth@tuolumnecounty.ca.gov for more information.
Important information to consider when selecting a third-party application:
Some third-party applications may request access to your private information and use it for their own purposes, such as distributing the information to secondary companies without your knowledge. Remember to research the applications that want access to your private health information and consider these questions:
- What are the ratings for this application?
- Does the website for the application look legitimate and well made?
- Does the application reference a clear privacy policy?
- Does the application come from a reputable company (i.e. Apple, Samsung, etc.)?
If you answer no to any of these questions when reviewing an application, it is probably not safe to use the application.
Your medical information is private information but there are some types of organizations or individuals that are not or may not be HIPAA-covered entities. Printed Access to your Health Record
Our Medical Records staff can assist you and/or your authorized representative with questions and requests regarding a copy of your health records managed by Tuolumne County Behavioral Health be sent to you or a third party.

To request your health records, for your convenience and record of request, you can fill out the form below and return it to Medical Records in one of the following manners:
- In person at 105 Hospital Road, Sonora Ca 95370;

Request for Access to Clinical Records
If you wish to release your health records to a third party for mental-health or substance-use-disorder services, please call us at (209)533-6245 option 0.

Our Provider Directory for Behavioral Health Services is updated monthly and list our current behavioral health providers within our system. The Provider Directory is available via PDF in English and printed versions are available upon request.
